Health Record Form Required For All Campers

A completed Xplorations Camper Health Record Form must be on file with the Xplorations Office prior to each child beginning camp.

Completed forms can be turned in three ways:

1. Fax your form to 713-639-4681

2. Emailed your form as an attachment to CLOAKING

3. Mail or drop off your form at this address:

Children will not be allowed to participate in camp activities without a completed Xplorations Camper Health Record Form on file prior to the first day of camp.

No refunds will be processed for activities missed due to a missing or incomplete Xplorations Camper Health Record Form.
